By Signing This Agreement, You Give Up Your Rights To File A Lawsuit To Recover Compensation Or Obtain Any Other Remedy For Any Personal Injury Or Property Damage However Caused Arising Out Of Your Minor Child’s Participation In Lakeside EPC VBS 2025, Now Or At Any Time In The Future. I Have Knowledge And Understanding Of The Inherent Risks Associated With Participation, Including But Not Limited To Slips, Falls, Athletic Injuries, And Illness, Including Exposure To And Infection With Viruses Or Bacteria. I Assume All The Foregoing Risks, Known And Unknown, And Accept Personal Responsibility For The Damages Following Such Injury, Permanent Disability, Or Death. Waiver, Release, And Indemnification. In Consideration Of My Participation In Lakeside EPC VBS 2025, I Agree To Release And, On Behalf Of Myself, My Heirs, Representatives, Executors, Administrators, And Assigns, Hereby Do Release Lakeside Presbyterian Church Its Officers, Directors, Employees, Volunteers, Agents, Representatives, And Insurers From Any Causes Of Action, Claims, Or Demands Of Any Nature Whatsoever Including, But In No Way Limited To, Claims Of Negligence Which I, My Heirs, Representatives, Executors, Administrators, And Assigns May Have, Now Or In The Future, Against Lakeside Presbyterian Church On Account Of Personal Injury, Property Damage, Death Or Accident Of Any Kind, Arising Out Of Or In Any Way Related To The Use Of Facilities/Equipment Of Or Participation In Lakeside EPC VBS 2025. By Typing My Full Name Below I Affirm And Understand The Permissions I Have Given Lakeside EPC Staff Regarding The Photographing And Health Of My Minor Child While Attending Lakeside EPC VBS 2025 And I Confirm That I Have Read The Waivers Above, Understand Them, And Agree To Be Bound By Them. I Understand That The Act Of Typing My Name Here Has The Same Legal Effect As Signing My Name On A Paper Agreement.
